The Marquardt District 15 Department of Health Services is dedicated to the promotion and maintenance of the health and wellness of the school community. The role of the health services staff is to promote the physical, mental, emotional and social well being of students with the goal of helping them achieve optimal learning.

Haemophilus Influenza Type b series (Hib) vaccine: One dose given after age 15 months

Pneumococcal conjugate series vaccine: Primary series complete, or 1 dose on/after 24 months of age
Kindergarten
Physical and eye exam due prior to October 15th of the current school year
Dental exam due prior to May 15th of the current school year

To report an absence, please call the attendance line of the student's school (below) and provide:
- The Student's Full Name
- School Grade
- The Student's Teacher (if Elementary)

Parent and Guardians: To report a student absence, press 7, to reach a nurse press 9. If we do not receive your student's absence, a school staff member will contact you.
Please note: Students who leave school due to illness will not be allowed to participate in after school activities.
FAQ: When should I keep my child home from school?

WHEN TO KEEP YOUR CHILD HOME FROM SCHOOL:

When student has had a fever of 100° or more within the last 24 hours.

When student has thrown up or had diarrhea within the last 24 hours.

When student has a rash that has not been seen by a doctor.
-
When student has pinkeye that has not been treated for a minimum of 24 hours.

As instructed by the local health department related to communicable (including COVID-19) and chronic disease guidelines. Parent/Guardian will need to bring in note stating student is able to return to school.
